Request a sick note
You will need to give your employer proof of illness if you have been ill for more than 7 days. This includes non working days such as weekends and bank holidays.
Please note that you can self-certify for the first seven days of illness-related absence. To obtain a statement of sickness, ask your employer for the necessary form, or visit gov.uk for more information.
If you think you need a note after 7 days of absence or self certification, please call the practice or request this through our online service which you can access via the home page of this website. We will take some details from you including a contact number as the GP may need to call you back. Whilst notes cannot be future dated, they can be backdated.
If you have been receiving notes for an ongoing complaint or if you have just been discharged from hospital and have a hospital letter the doctor may be able to give you a note without being seen.
