Request a sick note

“Sick notes” or Medical Certificates are now called Fit Notes. If you are off work for more than 5 working days or 1 calendar week your employer will ask you for one. GPs do not issue notes if you have not been off for this long. You can complete a “self certificate” which your employer should provide.

If you think you need a note please call your surgery. Our receptionist will take some details including a contact number as the GP may need to call you back. Whilst notes cannot be forward dated they can be backdated.

If you have been receiving notes for an ongoing complaint or if you have just been discharged from hospital and have a hospital letter the doctor may be able to give you a note without being seen. Please discuss this with the receptionist.
